If you are interested in serving me as a submissive, this is the best method I have found for determining whether we'll be compatible. It's also a great way to chase away curiosity seekers, time wasters and fundamentalists. My client application is divided into two parts. This is part one. To receive part two, you must fill out this first form and submit it to me. THAT is your first assignment, My dear. So give Me a glimpse of the 'real you' as you answer these questions; and make Mistress smile and laugh if you can!

I expect you to answer the questions below completely and to respond to the spirit of the questions -- answering them clearly and with more than 'yes' or 'no' responses. But BEWARE, because Mistress detests a liar! If we are to build a long term relationship it will be based on mutual trust and respect. So, please be honest with Me, My dear. Remember, a sense of humor goes a long way towards capturing My interest, as does respectful language, literacy and a sincere effort to show Me your personality.

When you have finished your application, submit it to Me on your knees. Then remain there patiently awaiting My instructions. If I deem you worthy and your responses are sufficiently submissive, I will respond... eventually!

Describe your general medical history. Information which could impact scenes would include old injuries, contact lenses or glasses, phobias, dental problems, heart, circulatory or neurological conditions, communicable diseases or infections (especially STDs past or present), back or joint problems, etc.

Describe your general psychological health. Areas of interest would be histories of abuse in family or relationships, medications taken, depression, use of controlled or recreational substances and whether any problems you recognize have been addressed in therapy or some other way. If you are aware of suffering from PTSD, for example, let me know that so I can avoid triggering responses which might jeopardize our respective safety.
